AARP COVID-19 Participant Liability Waiver & General Release

By submitting the form below, I agree that: 

• This form shall cover all AARP and AARP affiliated entities (collectively referred to as “AARP” hereafter) activities that I participate in described in the AARP Event Attended section at the bottom of this form.

• I assume all risk in connection with my decision to participate in support of or attend an AARP hosted, sponsored, and/or supported event and understand that the COVID-19 pandemic remains a threat to individual and public health, COVID-19 is a highly contagious disease transmitted through human contact and respiratory droplets and it is possible that I may contract COVID-19 while at an AARP hosted, sponsored, or supported event.

• The COVID-19 pandemic continues to evolve, as do the associated public health rules and guidance and AARP cannot guarantee my safety at AARP hosted, sponsored, and supported events and AARP cannot guarantee that attendees will comply with applicable guidance and requirements.

• I will comply with all AARP Safety Protocols, including without limitation, those regarding COVID-19.
  
  
• I will not attend an AARP in-person event, program or activity if within 14 days of the Event, I test positive for COVID-19 and/or experienced any COVID-19 symptoms, including without limitation, fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting and diarrhea.
  
  
• I will not attend an AARP in-person event, program or activity if I have been in close contact with a person who tested positive for COVID-19 in the last 10 days.

• I will use the CDC Coronavirus Self-Checker, or another self-checker as advised by AARP, on the day I am scheduled to attend each AARP in-person event, program or activity, before I arrive, and will not attend the AARP in-person event, program or activity unless I am cleared to do so by the CDC Coronavirus Self-Checker in each instance.

• If while in attendance at an AARP in-person event, program or activity, I experience or exhibit any COVID-19 symptoms I will immediately leave the premises.

• My personal information can be shared with health authorities for contact tracing purposes

• I will not be compensated for participating in the event or activity described in the AARP Event Attended section at the bottom of this form and AARP does not carry or maintain health, medical, disability or workers compensation insurance coverage for me.  

• I am physically capable of participating in each event or activity described in the AARP Event Attended section at the bottom of this form that I take part in.

• I release, discharge and hold harmless AARP and its affiliates, and their respective, employees, directors, officers, volunteers, vendors and agents from any and all liability, claims, and demands of every kind including without limitation, any bodily injury, illness, death, property damage or any other harm, that may arise from my participation.

• I grant AARP the right to photograph, record, videotape, live stream, use, and publish my image and voice, with or without my name, in whole or in part, individually or in conjunction with other materials, in any format, for the purpose of promoting AARP an unlimited number of times in perpetuity.

• In the event that any provision of this form shall be held to be invalid by any court of competent jurisdiction, the remaining provisions shall not be affected and shall continue to be enforceable.
